Case No. CR-2005-02027 NOTICE OF SUPPLEMENTAL PROBATION VIOLATION
COMES NOW the State of Idaho, by and through the Latah County Prosecuting Attorney, and hereby gives notice to the court and counsel of supplemental allegations that the defendant is in violation of the following conditions of his probation:
Condition number 13 of the Idaho Department of Correction Sexual Offender Agreement of Supervision — I agree to obtain a special sexual offender evaluation. The evaluator and my treatment provider must be clinical members of the Association for the Treatment of Sexual Abusers (ATSA), and approved by my supervising officer. I will comply with all requirements of the treatment program and actively participate in treatment until discharge is recommended mutually by the therapist and my supervising officer.
Condition number 17 of the defendant’s Valley Treatment Specialties Treatment Contract which provides, in part, — I understand fully and completely that I am required to make continuous progress through the treatment program.
These allegations are based on the June 18, 2007, letter from Dalton Lombard attached to the Report of Violation filed herein on June 19, 2007, and the attached Appendix “A,” Idaho Department of Correction Sexual Offender Agreement of Supervision and Appendix “B,” Valley Treatment Specialties Treatment Contract.
